Most conversations about blood sugar start too late, at the point of a diagnosis. But long before anyone is told they have diabetes, a quieter change has usually been building for years. It has a name that sounds technical but describes something simple: insulin resistance. Understanding it is one of the most useful things you can do for your long-term health, because it sits at the root of so much that follows.

The reassuring part is that, caught early, insulin resistance is often reversible. But you cannot address what you do not understand, so let's start with what is actually happening inside.

Insulin, the key and the lock

Every time you eat carbohydrate, it breaks down into glucose and enters your blood. Glucose is fuel, but it cannot get into your cells on its own. It needs a key: the hormone insulin, released by the pancreas. Insulin unlocks your cells so glucose can move out of the blood and into the muscles and liver, where it is used or stored. In a healthy system, this happens smoothly, and blood sugar stays in a narrow, steady range.

Insulin resistance is what happens when the locks stop responding well to the key. The cells become less sensitive to insulin's signal, so glucose lingers in the blood. The pancreas notices and responds the only way it can: by making more insulin. For a while, this extra insulin forces the doors open and keeps blood sugar normal. This is the crucial, sneaky phase. Your blood sugar can look completely fine on a routine test while your pancreas is quietly working overtime behind the scenes.

Why it stays hidden for so long

This compensation is exactly why insulin resistance is so easy to miss. For years, sometimes a decade or more, the pancreas keeps up, pumping out ever more insulin to hold blood sugar steady. You feel normal. A standard fasting glucose test looks normal. Nothing sounds an alarm. Trouble only becomes visible when the pancreas can no longer keep pace, and blood sugar finally starts to drift up into the prediabetes and then the diabetes range. By then, the underlying resistance has often been present for a very long time.

This is why insulin resistance is described as the root. It does not only lead to type 2 diabetes. The same underlying state is closely tied to fatty liver disease, to PCOS in women, to high triglycerides and blood pressure, and to the broader cluster doctors call metabolic syndrome. Address the root, and many of these branches improve together.

What drives it

Insulin resistance is not caused by one thing but by a handful of factors that add up. Excess fat stored around the organs, called visceral fat, is a major driver, and it can be present even in people who look slim. Physical inactivity matters, because muscles that are rarely used take up less glucose. Chronically poor sleep and high stress push in the same direction, as does a diet heavy in refined carbohydrate and sugary drinks. Genetics load the starting position, and everyday life does the rest.

South Asians deserve particular attention here. Research consistently shows that people of Indian origin tend to develop insulin resistance at a lower body weight than many other populations, often carrying more visceral fat for the same size. An Indian who looks perfectly healthy by the usual standards can still be insulin resistant. Being slim is reassuring, but it is not a guarantee, which is why the lifestyle factors matter for everyone, not only those who are visibly overweight.

The hopeful part: it responds

Unlike your genes or your age, insulin resistance responds to what you do. And the levers are unglamorous, familiar, and effective. Regular movement is perhaps the most powerful, because working muscles pull glucose from the blood directly and become more insulin sensitive over time, an effect that begins with the very first sessions. A modest, sustained reduction in excess weight, where relevant, has an outsized effect, especially on visceral fat. Cutting refined carbohydrate and sugary drinks while leaning on fibre, vegetables and whole foods eases the daily glucose load. And protecting sleep and managing stress remove two quiet forces that otherwise keep resistance high.

None of this requires an extreme regimen. It requires consistency in ordinary things, which is both the good news and the hard part.

Knowing where you stand

Because insulin resistance hides behind normal blood sugar for so long, it helps to look a little deeper than a single glucose reading. An HbA1c, a fasting glucose, a lipid profile, and where appropriate a fasting insulin, together paint a fuller picture than any one number alone. If you have a family history of diabetes, a larger waistline, high blood pressure, or PCOS, it is worth asking your doctor what testing makes sense for you.

Insulin resistance is not a diagnosis to fear. It is an early, honest signal, and one of the most actionable in all of health. The people who do best are simply the ones who understood it and acted while the door was still open.
